[보건] Telemedicine Readiness in Maldives for Pandemic Preparedness and Health Care Delivery in Atolls

Atoll health care has long been underinvested in the Maldives, and the provision of advanced health care has been concentrated in the capital, Malé. Patients from atolls must travel to the capital to receive appropriate care, further requiring referral to hospitals overseas. As a result, travel and associated costs have been the key obstacles in accessing adequate health care. At the height of the COVID-19 pandemic, such a centralized health care system has revealed its limitation that prompted the country to expand investments in atoll health care. From this vantage point, the role of telemedicine warrants exploration in providing advanced quality health care without transferring patients to central facilities. Telemedicine has the potential to strengthen the capacity of atoll health facilities. During the pandemic, telemedicine has quickly developed in providing routine consultation services or making decisions on COVID-19 patient transfers from islands to COVID-19 facilities. This development could provide an excellent opportunity to expand health care delivery in atolls, including specialized care for patients, by supporting hospitals and health centers through telemedicine. In response to the COVID-19 pandemic, the government has updated the Strategic Action Plan (SAP) 2019–2023, which is the key policy document for all sectors, highlighting five priority areas for health: (1) strengthening public health systems; (2) access to quality health care services in regions; (3) national ambulance service; (4) health care service digitization; and (5) medical procurement reform and expenditure efficiency. The government put a particular emphasis on telemedicine and digital health. However, it was unknown to what extent telemedicine was used and how prepared health facilities were for telemedicine. This study aims to assess the degree of development of telemedicine in the Maldives while describing its bottlenecks (for example, infrastructure, human resource, financing, organizational readiness, and regulatory and policy issues) and potential opportunities for expanding its role in delivering health care services in atolls and inhabited islands. The potential of expanding telemedicine at different levels will be examined with a focus on the regional and atoll hospitals and peripheral health centers in islands.
